Application Process

PLEASE NOTE: THE APPLICATION DEADLINE IS MONDAY, FEBRUARY 8, 2010. ELIGIBLE APPLICANTS WILL HAVE EITHER GRADUATED WITHIN THE PAST TWO YEARS OR WILL BE IN THEIR FINAL YEAR OF UNDERGRADUATE OR GRADUATE STUDIES AT ANY ACCREDITED FOUR-YEAR COLLEGE OR UNIVERSITY.

An application form must be submitted by all applicants. Forms are available from MDC at the address below, or can be downloaded here in PDF or MS Word format. Please click on the link for the application and save it to your computer.

INSTRUCTIONS FOR COMPLETING THE APPLICATION FORM: First, print a copy for reference; then, delete the lines (included on the form to indicate the length of the expected response) and use the blank spaces to complete pages 1-3 of the form, using 12-point font and preserving spacing and labels. If you send your application via email, please save it in the format Last Name, First Name Application (or Resume); for example: Zandt, Alyson Application.
